La Clinica de Familia is looking for dedicated individuals to fill the vacant positions listed below. HEALTHY START

Care Coordinator: Works with the Healthy Start Program to coordination health and social services for high-risk pregnant women (including teens), infants, and their families.

The Care Coordinator must be able to, asses consumer strengths and needs, develop and implement care plans, provide and follow-up on referral, and have a strong working knowledge of community resources. Duties include work with multi-disciplinary team to coordinate a broad range of services and support, conduct bio-psychosocial assessments and develop and monitor care plans and provide home visiting services.

The Care Coordinator must possess a solid working knowledge of child development and family life cycle, must be able to communicate with Spanish speaking consumers, excellent organizational and communication skills, ability to work with diverse populations, good computer skills and sound working knowledge of community resources. Applicant must be willing to travel, work independently, and provide home-based services.

Qualifications include: Bachelors Degree in Social Work or related health and social services field from an accredited school. (LBSW preferred).  A valid driver’s license and current automobile insurance required for travel. Bilingual English/Spanish preferred.

ADMINISTRATION

Nursing Director: Oversees and manages nursing functions at all medical clinics for LCDF. Supervises nursing staff, nurse recruitment and hiring, training & development, performance evaluation, employee issues, scheduling and work delegation, and related management functions. Ensures quality of care at all respective programs as well as full compliance with regulatory agencies (e.g., JCAHO) and all applicable policies and standards of performance. Provides the supportive, technical and/or management assistance necessary to effectively address organizational/operating problems, improve quality of services, and enhance service levels. Ensuring appropriate nursing staff levels. Provides nursing leadership and expertise for the organization and the community. Position requires BSN (MSN desirable) and at least three years managerial/administrative experience in healthcare setting as well as an exceptionally strong skill set in nursing management and leadership.

Dental Assistant (2 Positions): Serves as an intermediate between the dentist and the patient. The dental assistant maintains communication with the dentist, patient, and front office staff to ensure the patient receives satisfactory service and treatment. He/she assists the dentist chair side during treatment procedures and performs all assisting duties permitted by law. The Dental Assistant responsibilities include assisting the dentist with examinations and treatments and have knowledge of cleaning, sterilization, and arrangement of instruments. Candidate must assist dentist with x-rays. Qualifications include: High School Diploma and 6 months experience in a dental office and completion of a training course in dental assistance. Required License or Certificates: CPR certification and Dental Assistant Certification needed for this position. Bilingual English/Spanish is necessary.
