Chaparral

Rover Patient Representative: Maintains patient records according to applicable standards and provides support for overall operation of clinic's business office. Requires strong communication skills, efficient work practices, and exceptional interpersonal abilities. High School Diploma or equivalent with a minimum of one year’s clerical experience required; medical office or healthcare experience preferred. Bilingual English/Spanish is necessary.

Staff Nurse (RN): Provides direct nursing care and patient education at assigned family medical clinic. Nursing Director: Oversees and manages nursing functions at all medical clinics for LCDF. Supervises nursing staff, nurse recruitment and hiring, training & development, performance evaluation, employee issues, scheduling and work delegation, and related management functions. Reports to Chief Medical Officer. As member of senior management team ensures quality of care at all respective programs as well as full compliance with regulatory agencies (e.g., JCAHO) and all applicable policies and standards of performance. Provides the supportive, technical and/or management assistance necessary to effectively address organizational/operating problems, improve quality of services, and enhance service levels. Maintains key responsibility for ensuring appropriate nursing staff levels. Provides nursing leadership and expertise for the organization and the community. Position requires BSN (MSN desirable) and at least three years managerial/administrative experience in healthcare setting as well as an exceptionally strong skill set in nursing management and leadership.

Tobacco Use Prevention Specialist: Responsible for planning, coordinating and delivery of tobacco use prevention and cessation services and related health topics such weight management/nutrition, second hand smoke exposure to prenatal patients, infants and children. Bachelor’s Degree in Community Health or related field with two years of direct experience in tobacco cessation and prevention. Training in perinatal health is a plus. C.H.E.S. Certification is a plus. Bilingual English/Spanish.

Relevant work experience and knowledge of medical terminology are strongly preferred. Most important is ability to treat our patients with consideration, respect & compassion. High School Diploma or equivalent. Familiarity with Medical Terminology. Bilingual English/Spanish is necessary.

High School Diploma or equivalent and two years experience working in public social services.
Bilingual / Spanish desirable.

Triage Nurse (Member Services / Admin.): Working as a member of the new LCDF Call Center, responds to telephone requests for same-day appointments by determining urgency of caller’s needs. Utilizes specialized nursing skills and knowledge in assessing patient’s immediate treatments needs. Schedules appointments over the phone or makes other referrals as deemed appropriate. Other duties include: documenting and reporting on contacts, assessments and action plans; providing patient education; assisting with orientation of new staff; and helping other call center staff with incoming calls when not handling calls requiring triage. Position requires current NM Nursing License, completion of ADN or BSN from accredited nursing school, and working knowledge of Nurse Practice Act of New Mexico. Other requirements include: strong critical thinking skills; excellent communication skills, especially the ability to communicate effectively under pressure; exceptional interpersonal, customer service and organization skills; and fluent bilingual English/Spanish.

Health Educator: Responsible for planning, coordinating and delivery of a broad range of health education topics including prenatal and post-partum, diabetes, smoking cessation/prevention, weight management/nutrition, teen pregnancy prevention and parenting. Must be bilingual English/Spanish and have a Bachelor’s Degree in Community Health or related field with one year of direct experience.
